Individuals served by STAND, Inc. have the unique opportunity to access all services offered at STAND, Inc. no matter what program they enter through. There is “No Wrong Door” to accessing services.
If you or a loved one are dealing with substance uses issues, we are here to help. The Admissions process at STAND, Inc. is as easy as 1-2-3 for those that are eligible. To determine eligibility, you can simply download, complete, and send us the Referral form below. As soon as, you submit the form, you will hear from our Intake Department within 24-48 hours of your submission or on our next business day to complete a more detailed screening in order to determine your eligibility for one of our programs. Once eligibility is determined, you will be notified of your Intake date if space is available. If you are not accepted into one of our programs, you will be provided with the names, locations, and telephone numbers for alternative programs. If you are unable to complete the form, you can call us and we will complete the form via telephone.
Funding Options:
Medicaid
Medicaid Insurance is accepted for individuals that meet eligibility requirements for SAIOP or AD Peer Support.
State-Funded
STAND’s, SAIOP program also accepts Individuals without insurance and Individuals whose insurance does not cover substance abuse intensive services.
The Individual must:
- Meet the Admissions & Eligibility requirements for each program.
- Complete an Application for Financial Assistance for Community Mental Health and Addictive
Diseases form to determine whether they qualify for state financial assistance in paying for services. This application will be considered without regard to race, color, gender, age, handicap, religion, national origin, sexual orientation or political belief. Failure to provide accurate information may result in you being charged the full charge or in the denial of services.
- Provide Proof of Income such as:
Current Check Stubs, SSI, TANF, V.A. Benefits, Child Support, Alimony, Social Security Benefits, Retirement/Pension Payments, Trust Fund Payments, Other Regularly Scheduled Payments;
OR for individuals without income:
Social Security Earning Statement
Wage Inquiry from Department of Labor
Individuals whose adjusted income is at or below the poverty level for their family size, in accordance with Health and Human Services Poverty Guidelines, are determined not to have an ability to pay for services and therefore are not required to pay a fee.
Fee for Service
For individuals and families above the poverty guidelines, ability to pay is based on their adjusted income, family size and the percentage of the full charge payment as indicated on the DBHDD's sliding fee scale. Call us for more information.
SAMHSA Funded
STAND’s, PCT and PCP programs are both SAMHSA Funded and accepts individuals without insurance; however, Eligibility Criteria must be met.
